Required Documentation

To ensure eligibility and to begin the patient certification process, we require each patient to have a copy of their medical records to confirm your diagnosis.

You will need to provide us with a copy of your valid NYS Drivers License or Non-Driver ID, and a copy of your available medical documentation and/or records prior to your appointment.

Patient Intake

Our online process will gather patient identification and medical information. Medical documentation should document your qualifying medical condition. Be sure to include your current medical diagnosis, list of medications, any surgeries, or diagnostic tests confirming your qualifying condition(s).

Medical Records

If documentation is not available, please download the HIPAA Medical Release and Fax Cover Letter. Sign and complete and send directly to your Physician, who will then be able to send us your medical records on your behalf.

Proof of ID & Residency

We accept one of the following items as proof of ID & residency:

  • New York State Driver's License
  • New York State Non-Driver ID Card
  • Government-issued identification card that states your name and New York State address

Next Steps

Your certification information will be sent to you via email and USPS and is valid for 1 year.

Once you receive your certification from us, you will be able to register online for a medical marijuana card with the NYS Department of Health.

Once a patient or caregiver's registration is processed, the Department will mail a registry identification card (ID card) directly to the patient or caregiver. Registrations expire when the certification that was issued by the practitioner expires. This card must be shown when buying medical marijuana at a dispensing facility and carried by the eligible patient.
